Proximal Tibia Fracture: Causes, Symptoms, Treatment, and the Role of Physiotherapy

              • Introduction

A proximal tibia fracture is a break in the upper part of the tibia (shin bone), near the knee joint. Because this region plays an important role in weight-bearing and knee movement, these fractures can significantly affect walking, balance, and daily activities.

Proximal tibial fractures range from simple, stable fractures to complex injuries involving the knee joint surface. Early diagnosis, appropriate medical management, and structured rehabilitation are essential for achieving good functional recovery.


  What Is a Proximal Tibia Fracture?

The proximal tibia forms the lower part of the knee joint and includes the medial and lateral tibial condyles and the tibial plateau.

A fracture may involve:

·       Tibial plateau

·       Medial tibial condyle

·       Lateral tibial condyle

·       Anterior tibial region

·       Posterior tibial region

·       Metaphyseal region

·       Both medial and lateral condyles in severe injuries

·       Anterior and posterior parts of the tibial plateau in complex fractures.

 When the fracture extends into the knee joint surface, it is commonly referred to as a tibial plateau fracture.

Ø  Causes: 

Proximal tibia fractures can occur because of:

  1. High-energy trauma – such as road traffic accidents or falls from height.
  2. Sports injuries – particularly contact or high-impact sports.
  3. Low-energy falls – especially in older adults with osteoporosis.
  4. Twisting injuries – a sudden rotational force can damage the tibial plateau.
  5. Osteoporosis – weakened bone can fracture even after relatively minor trauma.

Ø  Signs and Symptoms

  • Severe pain around the knee and upper part of the shin
  • Swelling around the knee and upper tibia
  • Bruising or discoloration
  • Tenderness when touching the upper tibia
  • Difficulty or inability to bear weight on the injured leg
  • Pain while standing or walking
  • Difficulty bending or straightening the knee
  • Restricted knee movement and stiffness
  • Visible deformity in severe or displaced fractures
  • Instability or feeling that the knee cannot support the body
  • In severe injuries, numbness, tingling, or changes in foot circulation may occur  

  • In some cases, significant swelling or injury to surrounding structures may occur, making urgent medical assessment necessary.

    ü  Diagnosis

    Doctors generally assess the injury through a combination of physical examination and imaging.

    ü  X-ray

    X-rays are usually the first imaging investigation and can identify the location and pattern of the fracture.

    ü  CT Scan

    A CT scan may be recommended for complex fractures, particularly when the fracture extends into the knee joint. It helps clinicians understand the fracture pattern and plan treatment.

    ü  MRI

    MRI can be useful when there is concern about associated soft-tissue injuries, such as ligament or meniscal damage.


    v Treatment

    Treatment depends on the fracture pattern, displacement, joint involvement, soft-tissue condition, age, activity level, and overall health.

    Conservative Treatment

    Stable and minimally displaced fractures may sometimes be managed without surgery. Treatment can include:

      • Immobilization or bracing
      • Restricted or non-weight-bearing activity
      • Pain management
      • Regular imaging follow-up
      • Gradual progression of weight-bearing under medical supervision
      • Physiotherapy

     

    Ø  Surgical Treatment

      
    Surgery may be considered when there is significant displacement, joint-surface involvement, instability, or other complications. 

    Possible procedures include:

    • Open reduction and internal fixation (ORIF)
    • Plate and screw fixation
    • External fixation in selected severe injuries
    • Other procedures depending on the fracture pattern and patient factors

    The orthopedic surgeon determines the most appropriate approach.




    Ø  Role of Physiotherapy

    Physiotherapy is an important part of recovery following a proximal tibia fracture. Rehabilitation should be individualized and progressed according to fracture stability, surgical instructions, healing, and weight-bearing restrictions.

    Early Phase

    The goals are to:

    • Control pain and swelling
    • Maintain safe mobility
    • Prevent complications of prolonged immobility
    • Maintain movement in permitted joints
    • Preserve muscle activity

    Exercises may include ankle pumps, gentle muscle-setting exercises, and appropriate range-of-motion exercises when permitted.


    Intermediate Phase

    As healing progresses and the medical team allows increased movement and loading, rehabilitation may focus on:

    • Improving knee range of motion
    • Strengthening quadriceps and hamstrings
    • Improving hip and ankle strength
    • Progressive weight-bearing
    • Balance and proprioception
    • Gait training

    Advanced Phase

    Later rehabilitation may include:

    • Functional strengthening
    • Stair training
    • Dynamic balance exercises
    • Walking and movement retraining
    • Sport- or occupation-specific exercises when appropriate

    Possible Complications

    Some patients may experience:

    • Knee stiffness
    • Persistent pain
    • Muscle weakness
    • Reduced range of motion
    • Post-traumatic osteoarthritis
    • Malalignment
    • Delayed union or nonunion
    • Infection following surgery
    • Blood clots after significant lower-limb injury or surgery

    Regular follow-up is therefore important.

    Ø  Recovery

    Recovery varies considerably depending on the fracture type, treatment method, age, bone health, surgical procedure, and rehabilitation progress.

    Bone healing and functional recovery do not always occur at the same rate. A patient may require several months to regain strength, mobility, confidence, and normal function. Weight-bearing should not be increased simply because pain has decreased; it should follow the orthopedic team's instructions and evidence of healing.

    My name is Dansharonkumar Kunduru, a B.P.T. 1st-year student. One unexpected accident changed my life. I suffered a proximal tibia fracture and dislocation, with a small bone fragment piercing through my muscle and skin.

     I knew fractures from textbooks, but experiencing one taught me what pain truly means. Walking, standing, and even getting out of bed suddenly became difficult. Some days brought pain, frustration, loneliness, and fear about the future.

     But I learned something powerful:

     A broken bone does not mean a broken life.

     Recovery may be slow, but every small movement is a victory. Every day I heal, I become stronger—not only physically, but mentally and emotionally.

     My leg is injured, but my dreams are not.

    My body is healing, and my hope is growing.

    This is not the end of my journey—it is the beginning of my comeback.

     One day, I will walk again, look back at this painful chapter, and say:

     

    “I survived it. I healed. And I came back stronger.” This is my journey of healing, patience, courage, and coming back stronger.
